West Nile Virus Surveillance System Analysis Questionnaire
Some AAZV members may have been contacted or soon will be contacted to participate in the evaluation of the West Nile Virus Surveillance System that was sponsored by Centers for Disease Control and AZA.  The program was administered by Lincoln Park Zoo, and diagnostics were handled by Cornell Veterinary Diagnostic Laboratory.  The program ran from 2001-2006 and provided free testing for West Nile Virus.  You may have been the designated Contact Person for your institution.
This survey is an important part of the evaluation of the surveillance system itself.  I am using Updated Guidelines for Evaluating Public Health Surveillance Systems, a CDC guideline, to evaluate the performance of the WNV system.
Even if your institution only submitted a handful of samples, your experience is important.  If you elected not to submit many samples, a bad experience with the program may have decreased your participation.  We want to know what happened.
I would like to encourage you to participate in this survey.  Your comments are vital in understanding the value that the program had to the community.  It is also important to understand what the problems were, from the perspective of its users.
If you have any questions about the survey, or if you feel that you should contribute information about the program but haven’t yet received an invitation, please let me know by contacting me via e-mail or by phone at 312-742-6600.  Thank you.
