Application for a permit to import animal products (Blood products, tissue, urine, rumen contents, feces etc) 

nb: Most carnivores, all primates, all reptiles (except for 3 tortoises and small turtles) do not require USDA permits for importing samples 

To apply for an animal product import permit, you can access the USDA APHIS website and look under permits, Apply for VS permit (in main body of text), and scroll to the end of the page for manual application. If you plan on doing this regularly, then you may consider the e-registration process. Blood and blood products will require VS 16-3 permit which costs $133 and can be paid by credit card over the phone.
